We’re excited to have you as part of Dr. Samadi’s Health & Wellness Institute. Below you’ll find an overview of how your membership works, what’s included, and how to make the most of it.

🕒 Clinic Access & Logistical Considerations:

  • Clinic hours: Monday–Friday, 9:00 AM–3:00 PM.
  • We follow FWISD for inclement weather conditions.
  • Closed during major holidays.
  • All visits are by appointment only.
  • Same-day appointments will be offered for urgent medical cases, when possible.
  • Telemedicine visits may be arranged, depending on the nature of your medical needs.
  • Medical services are available only during business hours.
  • No after-hours text, telephone, or email access is available.
  • All in-person services will be provided at Dr. Samadi’s Health & Wellness Institute.
  • Telemedicine or in-office visits during non-clinic hours (evenings/weekends) may be arranged, when necessary and possible; additional fees apply.
  • Limited off-site medical visits may be arranged, when necessary and possible; additional fees apply.
  • Members are strongly encouraged to maintain a relationship with their primary care physician (PCP).
  • Members are strongly encouraged to maintain their health insurance coverage.
  • The Institute does not have agreements with hospitals and cannot provide in-patient services.
  • The Institute does not have agreements with other medical providers or healthcare centers or hospitals.
  • The Institute does not contract with governmental or third-party insurance companies.
  • Staff cannot assist with insurance claims, payments, pre-authorizations, or related paperwork.

💎 Membership Benefits

  • Each member receives one (1) free office visit per month.
  • Additional visits within the same month are available at discounted rates.
  • Members receive discounts on:
  • Wellness programs (e.g., weight management, BHRT hormone replacement/optimization).
  • Laboratory testing (through Quest Diagnostics).
  • Diagnostic imaging services (X-ray, CT, MRI), when available.
  • Pharmaceutical-grade supplements.
  • Aesthetic medicine treatments (e.g., Botox, dermal fillers, lasers, microneedling, facials, and other cosmetic services).

📋 Membership Terms

  • A minimum of three (3) month commitment, payable in advance, is required.
  • Discounted payment plans of six (6) and 12 (twelve) months are also available.
  • After the initial commitment, membership may be canceled with 30 days’ written notice. Any prepaid balance will be prorated and refunded.
  • All past-due membership or medical fees must be paid in full for services to continue.
  • Appointment cancellations, barring emergencies, require at least 48 hours’ notice.
  • A $50 cancellation fee will apply for late cancellations or no-shows (except in emergencies).
  • If necessary, membership fees can be financed through Care Credit, finance fee will apply.
